Matte out each part you want to put in a fill background using Dstover and Misc. Once that's complete you can matte out the center, composite an image then output it as a GIF and take it to Dave's shadow tool.
When done, click it on and then re-enter the url back into the shadow tool but this time just change the box at the bottom and click on Top Left. This will put a shaded border around the left and top side so it now looks like a frame.
Take it back to I.M. and optimize and output as a jpg.
Here are the numbers for the path drawing. Just cc&p and put in the coordinate box. Just use a stroke color of a width of 2 (no fill).
